Submitting Completed Pharmacy Vouchers to CSA
Completed pharmacy vouchers must be submitted to the CSA on a monthly basis. Vouchers should be sent in
as part of your monthly prescription form submission but must be kept as a separate section and the number of vouchers recorded against section “H – Pharmacy Vouchers” on the HS30. It is important that vouchers not be mixed in with HS21 prescription forms as this may cause delays in payment or inaccurate payments.
