To recieve a replacement Medical Card you can fill in a form HS100, which is available from your doctor’s surgery, from CSA Franklin Street or by clicking the above link. A replacement medical card will normally be sent to you within 3 weeks but if you need one sooner you should mark your application “URGENT” |
