Northern Ireland medical card:

Application for inclusion in a practice list and/or health service registration

If coming into Northern Ireland from outside the UK then application form HS22X is required in order to obtain a Medical Card. You can download the form in its English and translated forms by clicking here

If coming to Northern Ireland from within the United Kingdom then your GB medical card will be sufficent to register with a doctor. If you have lost, misplaced or never received such a card then form HS200 can be used to register. Click the above link to view and print a HS200.

If seeking a replacement Medical Card you can fill in a form HS100, which is available from your doctor’s surgery, from BSO Franklin Street or by clicking the above link.

A replacement medical card will normally be sent to you within 3 weeks but if you need one sooner you should mark your application “URGENT”
