Click on the link to download a copy (pdf Format) of the Holiday Oxygen Scheme form (HOOF). Please bring the form to your GP to complete. The form should then be returned to the address given below. Alternatively the GP may fax the completed form to 028 9053 5634. You should also confirm, if travelling by ferry or airplane with the transport company are prepared to allow you to bring oxygen cylinders on-board. If you intend staying at a hotel, please ensure that the manager is prepared to allow oxygen equipment to be stored or installed in the hotel.
