Registering for Health Service Treatment

- bring medical card, issued by Central Services Agency, to dentist on your first visit

- agree with dentist if you will be registered as a Health Service patient

- agree Health Service and private treatment option

- request a treatment plan and estimate of cost (Form HS45DC or equivalent)

Complaints

If you have a complaint about Health Service treatment you should in the first instance take the matter up with the dentist. If you are not satisfied with the outcome you may write to the Complaints Officer at the relevant Health and Social Services Board.
