- you or your partner are named on a valid Pension Credit Guarantee Credit Certificate

If you are a pensioner and you or your partner are named on a valid Pension Credit Guarantee Credit Certificate, you are exempt from paying the prescription charges.

- you or your partner are named on a valid Pension Credit Guarantee Credit Certificate

If you are a pensioner and you or your partner are named on a valid Pension Credit Guarantee Credit Certificate, you are entitled to a voucher towards the cost of the glasses.

- you or your partner are named on a valid Pension Credit Guarantee Credit Certificate

If you are a pensioner and you or your partner are named on a valid Pension Credit Guarantee Credit Certificate, you are entitled to free eyesight tests.

- you or your partner are named on a valid Pension Credit Guarantee Credit Certificate

If you are a pensioner and you or your partner are named on a valid Pension Credit Guarantee Credit Certificate, you do not have to pay for your NHS Dental treatment.

- you are under 16 years of age

If you are registered for Health Service treatment and you are under 16 years of age you are entitled to a voucher towards the cost of glasses.

- you are 16, 17 or 18 and in full-time education

If you are registered for Health Service treatment and you are 16, 17 or 18 years of age and in full-time education you are entitled to a voucher towards the cost of glasses.

- you or your partner are in receipt of Income Support or Income-based Job Seekers Allowance

If you are registered for Health Service treatment and you or your partner are in receipt of Income Support or Income-based Job Seekers Allowance you are entitled to a voucher towards the cost of glasses.

- you are in receipt of a valid NHS Tax Credit Exemption Certificate

If you are registered for Health Service treatment and you are in possession of a valid NHS Tax Credit Exemption Certificate, your are entitled to a voucher towards the cost of glasses.

- you are named on a valid NHS Low Income Scheme HC2 certificate

If you are a registered patient with a General Practitioner in the UK and you are named in a valid NHS Low Income Scheme HC2 certificate you are entitled to a voucher towards the cost of glasses.

- you require complex or powerful lenses

If you are registered for Health Service treatment and you require complex lenses or powerful glasses with at least one less which:

· has a power in any one meridian of + or - 10 or more dioptres OR

· is a prism controlled bi-focal lens,

You are entitled to a special voucher towards the costs of glasses.

- you are under 16 years of age

If you are registered for Health Service treatment and you are under 16 years of age, you are entitled to free eyesight tests.

- you are 16, 17 or 18 years old and in full-time education

If you are registered for Health Service treatment and you are 16, 17 or 18 years of age and in full-time education you are entitled to free eyesight tests.

- you are 60 years old or over

If you are registered for Health Service treatment and you are 60 years of age or over you are entitled to free eyesight tests.

- you or your partner are in receipt of Income Support or Income-based Job Seekers Allowance

If you are registered for Health Service treatment and you or your partner are in receipt of Income Support or Income-based Job Seekers Allowance you are entitled to free eyesight tests.

- you are named on a valid NHS Tax Credit Exemption Certificate

If you are registered for Health Service treatment and you are named on an NHS Tax Credit Exemption Certificate, you are entitled to free eyesight tests.

- you are named on a valid NHS Low Income Scheme Certificate

If you are registered for Health Service treatment and you are named on a valid NHS Low Income Scheme (HC2) certificate you are entitled to free eyesight tests. You can apply for a HC2 certificate by completing a Form HC1 available from your Doctor, Dentist, Optician or Social Security Office. You should send it to your local Social Security Office on completion where your entitlement will be assessed. Further advice and explanation is on the HC1 Form.

- you are a diagnosed diabetic or you suffer from Diabetes Mellitus

If you are registered for Health Service treatment and you are a diagnosed diabetic or you suffer from Diabetes Mellitus (except where treatment is by diet alone), you are entitled to free eyesight tests.

- you have diagnosed glaucoma

If you are registered for Health Service treatment and you have diagnosed glaucoma you do not have to pay for your eyesight tests.

- you are over 40 years and you have been advised that you are at risk of glaucoma or a member of your immediate family has diagnosed glaucoma

If you are registered for Health Service treatment and you are over 40 years of age and a member of your immediate family has diagnosed glaucoma or if you have been advised by an ophthalmologist that you are at risk of glaucoma you are entitled to free eyesight tests.

- you are registered blind or partially sighted

If you are registered for Health Service treatment and you are registered blind or partially sighted you are entitled to free eyesight tests.

- you require complex lenses or powerful glasses

If you are registered for Health Service treatment and you require complex lenses or powerful glasses with at least one less which:

· has a power in any one meridian of + or - 10 or more dioptres OR

· is a prism controlled bi-focal lens,

You are entitled to free eyesight tests.

- you are under 18 years of age

If you are registered for Health Service treatment and you are under 18 years of age you do not have to pay for your NHS Dental treatment.

- you are 18 years of age and you are in full-time education

If you are registered for Health Service treatment and you are 18 years of age and you are in full-time education you do not have to pay for your NHS Dental treatment.

- you are pregnant or you have had a baby in the previous 12 months

If you are registered for Health Service treatment and you are pregnant or you have had a baby in the previous 12 months and you are named on a valid Maternity Exemption Certificate issued by the Central Services Agency, you do not have to pay for your NHS Dental treatment.

- you or your partner are in receipt of Income Support or Income-based Job Seekers Allowance

If you are registered for Health Service treatment and you or your partner are in receipt of Income Support, or income-based Job Seekers Allowance you do not have to pay for your NHS Dental treatment.

- you are named on an NHS Tax Credit Exemption Certificate

If you are registered for Health Service treatment and you are named on a valid NHS Tax Credit exemption certificate, you do not have to pay for your NHS Dental treatment.

- you or your partner hold a current HS Charges Exemption Certificate

If you are registered for Health Service treatment and you or your partner hold a current HS Charges Certificate you may be exempt from paying all or part of your NHS Dental treatment costs. You can apply for a HS Charges Certificate by completing a Form HC1 available from your Doctor, Dentist, Optician or Social Security Office. You should send it in to your local Social Security Office on completion where your entitlement will be assessed. Further advice and explanation is on the HC1 Form.

- you are under 16 years of age

If you are registered for Health Service treatment and you are Under 16 years of age you are exempt from paying for your prescriptions.

- you are 16, 17 or 18 years old and in full-time education

If you are registered for Health Service treatment and you are aged 16, 17 or 18 and you are in full-time education you are exempt from paying from your prescriptions.

- you are 60 years of age or over

If you are registered for Health Service treatment and you are 60 years of age or over you are exempt from paying for your prescriptions.

- you are named on a valid Maternity Exemption Certificate

If you are registered for Health Service treatment and you are pregnant or you are a Nursing Mother (whose baby was born in the previous 12 months) you should complete Form HC 11B, available from your Doctor, Midwife or Health Visitor to apply for a prescription payment exemption certificate.

- you are named on a valid Medical Exemption Certificate

If you are registered for Health Service treatment and you suffer from one of the medical conditions which would entitle you to free NHS prescriptions, or if your most recent certificate has expired, you should complete Form HC 11B, available from your Doctor, Hospital or Pharmacy to obtain a prescription payment exemption certificate.

- you are named on a valid pescription Pre-Payment Certificate

If you are registered for Health Service treatment and you also hold a current prescription prepayment certificate, available at pharmacies, you are exempt from payment of the prescription charges.

- you are named on a valid War/MoD exemption certificate

If you are the holder of a UK Medical Card and you also hold a War Pension exemption certificate you are exempt from payment of the prescription charges if the treatment is for your pensionable disability.

- you or your partner are in receipt of Income Support

If you are registered for Health Service treatment and you, are in receipt of Income Support you, and your partner if named on the claim, are entitled to free NHS prescriptions.

- you are named on a valid NHS Tax Credit Exemption Certificate

If you are registered for Health Service treatment and you are in receipt of a valid NHS Tax Credit Exemption Certificate issued by the Inland Revenue, you are exempt from payment of the prescription charges. To enquire about your entitlement or to apply for a certificate, you should contact the Inland Revenue.

- you or your partner are in receipt of income-based Job Seekers Allowance

If you are registered for Health Service treatment and you are in receipt of income-based Job Seekers Allowance you, and your partner if named on the claim, are entitled to free NHS prescriptions.

- you are named on a valid HC2 charges certificate

If you are registered for Health Service treatment and you are named on a current NHS Low Income Scheme HC2 Certificate you are exempt from payment of the prescription charges. If you feel that you may be entitled to a HC2 Certificate you should contact your local Social Security Office.

- you were prescribed a free-of-charge contraceptive

If you are registered for Health Service treatment and you have been prescribed Contraceptives, the prescription charges will not apply to these items.
